Probably you upgrade not only my extension.
For example, you installed some other extension.
Set in component options Map Initialization Type to JQuery domready event
Give direct link to map page, I'll check it.

Check you have the latest versions of all parts of extension (Extensions->Manage->Update->Clear cache, Find updates)

- check browser console for errors
- set jQuery initialization map type
you can also mail me your page (you can save it as is in browser)
just save it and mail me zip file with your page and assets
