Display and more problems

hi all i have a problem with this GREAT at all extention...
when i put the plugin code in my wamp website i have some problems j3.15

1) if i klick in icon cant close it dont have close icon (see atachments)
2) display weird elements in the map (see atachments)

how can i fix this ?
the same problems i have and in j25 when i have make update the component and plg
