Map module not appearing with Menu item of Category List

I followed your approach and entered the settings via the Menu items entry point instead of via the Extensions/Module method.

As far as I can see both methods give you the same choices as far as Assignments go.

I've attached two of the Menu details - one that works and one that doesn't.
I also attached views of those two Menu items showing the overview of the Module Assignments

Looks to me like the settings are the same - that's why I am confused as to what I'm missing.
You can see the Menu option in the background at top left of each pic.

I have no idea.
